Sue was the teacher of the deaf of the first child in the UK to have a cochlear implant. She then helped establish The Ear Foundation to fund the first paediatric cochlear implants in the UK, and went on to co-ordinate the Nottingham Paediatric Cochlear Implant Programme from its inception in 1989 until 2004. There she developed methods of assessing and monitoring young children for implantation and a database to manage a cochlear implant programme. She has published widely on the education of deaf children and on outcomes from implantation, and recently received her doctorate from the University of Nijmegen, cum laude, on the subject of Deaf Education: changed by cochlear implantation? She is now Chief Executive of The Ear Foundation, leading its programme of support, information, education and research to ensure the maximum benefit from the latest hearing technologies at home, school and work.

My name is Dr Nur Af’Idah M. Zulkefli. I am a 2nd year post graduate student from Department of Otorhinolaryngology, Universiti Kebangsaan Malaysia Medical Centre (UKMMC), Malaysia.
I am doing a dissertation with the topic of: Benefits of Cochlear Implant in Children with Multiple Disabilities: Audiological Outcome and Speech Performance. My supervisor is Prof Dr. Goh Bee See from UKMMC.
My general objective is to assess the benefits of CI in children with MD in comparison to profound HL children without disabilities. I will be using CAP-II score, SIR rating scale and PEACH (Parents Evaluation of Aural/Oral performance of Children) as my study instrument. Besides that, I also will examine benefits of CI in this group of children in term of daily life activities by using questionnaires for parents/ guardian to answers.
I am requesting your permission to use the Categories of Auditory Performance (CAP) in my dissertation.

My name is Joice, I’m a Master Student in speech-language pathologist post-graduate program from School of Dentistry at University of São Paulo, under supervising of Prof Dr. Adriane Lima Mortari Moret.
In my research to obtain the master’s degree, we used the questionnaire “Children With Cochlear Implants: Parent’s Perspectives” elaborated by Archbold et al. (2002), translated and validated to Portuguese by Fortunato-Tavares et al. (2012). However, at the moment of data scoring and analyzes, we noticed that the data base “Parent Views and Experiences Questionnaire Data Entry (version 1.02: ISVR Software, Copyright 2003)” elaborated by the team of Ear Foundation (http://www.earfoundation.org.uk/) and previously used in published articles are not available for online access.
Our difficulty is to find articles with guidelines regarding the analyses and the detailed classification of the questions as quantitative, qualitative, positive, and negative. Therefore, we ask, if possible, instruction and recommendations about the appropriate manner to score the questionnaire.
